If you are over the age of 18, you will need a government-issued photo ID such as a passport, driver’s license or military ID. Birth Certificates will NOT be accepted.
If you are under the age of 18, you will need your government-issued photo ID, along with your guardian’s government-issued photo ID. BOTH LAST NAMES AND ADDRESS MUST MATCH.
